Required Registration up to 200 attendees
Whether your state's Codequest course will be held live in-person or virtually via video conference, this option is required. Course fee includes registration for up to 200 participants.

Option In-Person Instructor Travel Fee
States hosting a live, in-person Codequest course are required to select this option. Fee includes your instructor's round-trip airfare, ground transportation and food. No exceptions, exemptions or invoice arrangements. This is the only option for bringing a live instructor to your location in the contiguous United States. Societies are responsible for securing a hotel room for a traveling instructor.

Extra 201 or More Attendees
If final attendance for your Codequest course exceeds 200 participants, you must return to this order page and account for the additional number of participants here. (For example, if 296 people attended your state's course, add qty 96 units to the cart and complete your order on the checkout page.)

Extra Bonus Hours
Some states request additional hours of coding instruction. Contact the Coding Team to schedule your Bonus Hours BEFORE placing an order for this option. Bonus Hours must be scheduled within one day of your Codequest Course.


Terms & Conditions — Codequest + Fundamentals Course


I am purchasing the selected number of 3-hour Fundamentals of Ophthalmic Coding course + 3-hour Codequest course from the American Academy of Ophthalmology at the cost of $8250 for both courses in one day.

Sponsor Responsibilities

Promotions
The Society will be responsible for all aspects of registration, event promotions and marketing.

Location/On-site Responsibilities

  • The Society shall select and pay for seminar site(s) appropriate to facilitate the needs of the course.
  • The Society is responsible for all management costs and duties, such as registration, suitable seating, catering and audio/visual equipment arrangements.
  • The Society is responsible for distributing to their attendees, the course workbook received electronically from the Academy coding program specialist. 

Eligibility Requirements
Attendance is limited to members of the Academy and/or the Society and those who are directly employed by ophthalmologists.

Registration

The Society sets and collects any registration fees. 

The Society will forward the final list of all registered paid participants to the Academy coding program specialist within five (5) business days of the course conclusion.

Registration list will include: 

  • Participant First Name,
  • Participant Last Name,
  • Professional Title,
  • E-mail Address

If the final attendance count exceeds 200 participants for any course, the Society will pay an additional participant fee of $60 per additional participant per course.

Instructor Housing
The Society will book instructor hotel arrangements, pre-pay for the instructor’s room up to two (2) nights and provide the instructor with hotel confirmation ten (10) business days prior to the course.

Standard Course Schedule for 3-hour Fundamentals of Ophthalmic Coding course and 3-hour Codequest course in one day:

Course 1

  • 07:00 – 08:00 am Registration/Breakfast
  • 08:00 – 9:30 am Course begins
  • 09:30 – 9:45 am Break
  • 09:45 – 11:15 am Course continues
  • 11:15 am Course 1 concludes

Course 2

  • 11:15 am – 12:15 pm Course 2 Registration/Lunch
  • 12:15 pm – 1:45 pm Course 2 begins
  • 1:45 pm – 2:00 pm Break
  • 2:00 pm – 3:30 pm Course 2 continues
  • 3:30 pm – Course 2 concludes

The Society will notify the Academy immediately if the Society needs to alter the course schedule above.

Staffing and Attendees 
The Society is responsible for providing the staffing necessary for onsite course registration and logistics.

Cancellation/Reschedule Policy

Both the Academy and the Society will make every effort to schedule course dates to avoid potential travel problems.

If the instructor does not arrive to the site due to airline conflicts, inclement weather or medical emergency, the Academy reserves the right to substitute an instructor to deliver the course at the date specified or to reschedule an acceptable date with the Society if a qualified substitute instructor cannot be located. The Academy will not be responsible for any expenses that might have been incurred by the Society. 

Should the Society need to cancel their scheduled course, the Academy will return the course payment amount to the Society if the Academy receives written cancellation from the Society within thirty (30) days from the purchase date. Cancellations received after 30 days from the purchase date and/or within 30 days of the Society’s scheduled course date will result in a forfeit of the payment unless an alternate date can be mutually agreed upon.

Notwithstanding the foregoing, if the Society or the Academy have to cancel the course because the use of or ability to travel to said premises are prevented, seriously impaired or rendered impracticable by Act of God, bankruptcy, strike, lockout, labor disturbances, explosion, sabotage, actual war, domestic terrorism, civil riot, outbreak of disease or other health conditions, fire, severe weather, flood, hurricane, earthquake, other casualty, political or social boycott, change in state/local law, or by any restrictions imposed by any governmental authority, then this Agreement shall be null and void and of no effect and there shall be no further liability on the part of either party. Under these force majeure conditions, payment will be returned. The parties can agree to enter into a new agreement for the course at a later date.

The Academy reserves the right to continue to hold this course independent of the Society if the Society cancels.

Disclaimer and Limitation of Liability

The Society agrees to the Disclaimer and Liability language in this section, and it further agrees to include this language, (A) and (B), in its registration materials for both advance and onsite registration.

A. The American Academy of Ophthalmology specifically disclaims any and all liability for errors in, omissions from, or use of any information provided by the American Academy of Ophthalmology, its employees, agents or representatives, including but not limited to, injuries or other damages of any kind for any and all claims that may arise out of the use of any idea, technique or method demonstrated or described in any material by any presenter and/or author, whether such claims are asserted by a physician or any other person.

B. All information provided by the Academy, its owners, employees, agents, or representatives who participate in the Academy’s coding course is based on information deemed to be current and reliable. The Academy does not provide legal or accounting services or advice, and the Society and the course attendees should seek legal and/or accounting advice as appropriate to them, their owners, employees, agents, or representatives. Coding and reimbursement is a complicated process involving continually changing rules and the application of judgment to factual situations. The Academy does not guarantee or warrant that either public or private payers will agree with its opinions. The Academy shall not be liable to the Society and course attendees to any extent whatsoever for inadvertent errors in or omissions from any such information provided by the Academy, its employees, agents or representatives. The Academy’s sole liability for providing the coding information shall be limited to the amount paid to the Academy for the coding course.

Interpretation of Contract

The American Academy of Ophthalmology shall have the full authority to interpret and amend all policies, terms, rules and regulations contained in any part of the Contract, and its decision as to the meaning and implementation of a policy, term, rule or regulation is final. The Society agrees to abide by any policies, terms, rules or regulations that may hereafter be adopted, which shall be as much a part of the Contract as though originally incorporated. In the event the Academy changes any policy, term, rule or regulation after the acceptance of the Contract by the Society, the Academy will provide written notice to the Society. If the Society objects to the change, the Society must notify the Academy within ten (10) business days of the date of the Academy’s notice of its intent to cancel the Contract, in which case the Academy’s sole obligation will be to refund any deposits of the Society. The failure of the Academy to enforce at any time any provision of this Contract shall not be construed to be a waiver of such. This Contract shall be governed by and construed in accordance with the laws of the State of California, exclusive of California’s choice of law provisions. All claims, disputes and other matters in question between the Academy and the Society arising out of or relating to this Contract or its breach shall be decided by binding arbitration in San Francisco, California, in accordance with the Commercial Arbitration Rules of the American Arbitration Association then in effect. Notice of demand for arbitration shall be filed in writing with the other party and with the American Arbitration Association. The arbitrator’s award shall be final, and judgement may be entered upon it in accordance with applicable law in any court having jurisdiction over it. The prevailing party in any dispute with respect to the meaning or enforceability of this Contract, or the enforcement of any provisions thereof, whether or not said dispute results in arbitration or litigation, shall recover from the other party all reasonable costs and expenses, including, without limitation, reasonable attorneys’ fees.

Assignment, Successors and Assigns

This Contract shall be binding upon the Academy and the Society and their successors and assigns. No Society may assign or transfer any rights or obligations under this Agreement without the written consent of the Academy. No such assignment shall release the Society from its obligations and liabilities under this Contract.

The sponsor warrants and represents that there are no outstanding legal, financial or governmental actions that could negatively impact their ability to host. If the accuracy of this representation and warranty changes before the Codequest event date, the Society agrees to provide immediate written notice to the Academy.

The Academy reserves the right to accept or reject, in its sole discretion, any purchase order to host Codequest and to determine the eligibility of any Society.
